TAVI in 2020: Not Just for High-Risk Patients

Learning Outcomes:
1. Describe the key pathological features of severe aortic stenosis
2. Describe basic investigations and triggers for referral
3. Explain the management options
4. Describe the concept of the heart team
5. Discuss relevant issues to the patient demographic including life expectancy, frailty and measures of futility
6. Explain the burden of valvular heart disease in an ageing population
7. Describe common complications and considerations of treatment options when moving to lower risk populations

Abstract:
Transcatheter aortic valve implantation (TAVI) has generated a worldwide paradigm shift in the treatment of severe symptomatic aortic stenosis (AS) over the past decade, particularly for high and intermediate-risk patients. Earlier in 2019, the first two randomised controlled trials (RCTs) comparing the outcomes of surgical aortic valve replacement (SAVR) or TAVI in low surgical risk cohorts were published. Their findings and implications create future challenges for Heart teams in Australia and New Zealand. A collaborative and multidisciplinary approach to patient selection and management is required involving primary care to ensure optimal patient outcomes.
