This content will be used for educational purposes or promotion of educational material and your profile on the Connect The Docs websites, including but not limited to, MEDD, MEDD Linkd, MEDD Live education portal, Hope For Hearts, and their marketing and SEO strategy. 

Connect the Docs does not accept responsibility for the accuracy of the content provided nor does it accept liability for any claims made against its use outside of being published on the Connect the Docs website and SEO.

Media releases will be submitted in accordance with a brief and approved by the relevant people before submission. It is acknowledged that while Connect The Docs and any of its staff (“The Contractor”) will use reasonable endeavours to procure publicity for the Client, the Contractor does not guarantee that any media coverage or another benefit will result from the Contractor’s activities of submitting the media release.

By signing this agreement, I agree to release, defend, and hold harmless Connect the Docs and its agents or employees, including any firm publishing and/or distributing the finished product in whole or in part, whether on paper, via electronic media, or on Websites, from any claim, damages, or liability arising from or related to the use of the photographs/video, including but not limited to any misuse, distortion, blurring, alteration, optical illusion, or use in composite form, either intentionally or otherwise, that may occur or be produced in taking, processing, reduction, or production of the finished product, its publication, or distribution.

I am 18 years of age or older and have read this release before signing below, fully understanding the contents, meaning, and impact of this release. I understand that I am free to address any specific questions regarding this release by submitting those questions in writing prior to signing, and I agree that my failure to do so will be interpreted as a free and knowledgeable acceptance of the terms of this release.
